Treatment Policy

Thank you for choosing us for your PMU or Tattoo Removal treatment. 1. Booking, Consultation & Intake Form Before undergoing any PMU Removal or Laser Tattoo Removal treatment, all clients are required to complete the PMU & Tattoo Removal Intake Form. A consultation is required before treatment to assess your tattoo or permanent makeup, discuss your expectations, review your medical history and determine whether the treatment is suitable for you. 2. Non-Refundable Deposit A non-refundable deposit is required to secure your appointment. Appointments are not confirmed until the deposit has been received. 3. Cancellations & Rescheduling We kindly request a minimum of 48 hours' notice if you need to cancel or reschedule your appointment. Cancellations made with less than 48 hours' notice will result in the loss of the deposit. Failure to attend your appointment without notice (no-show) will result in a charge of 100% of the scheduled treatment fee. 4. Late Arrivals Please arrive on time for your appointment. Clients arriving more than 15 minutes late may need to have their appointment shortened or rescheduled. In such cases, the deposit may be forfeited. 5. Medical Conditions & Contraindications Clients must disclose all relevant medical conditions, medications, allergies and skin sensitivities before treatment. Certain conditions, including pregnancy, active skin infections, some medical conditions or medications, may prevent treatment or require postponement. Failure to provide accurate information may result in treatment being refused without refund. 6. Age Requirement All clients must be 18 years of age or older. 7. Treatment Expectations Laser Tattoo Removal and PMU Removal usually require multiple sessions depending on factors such as pigment type, tattoo size, colour, depth, age of the tattoo and individual skin response. Results vary from person to person, and complete removal cannot be guaranteed. Your practitioner will discuss realistic expectations and an estimated treatment plan during your consultation. 8. Aftercare Following the recommended aftercare instructions is essential for proper skin recovery and to achieve the best possible outcome. Failure to follow aftercare advice may affect healing and treatment results. 9. Refund Policy Due to the nature of these treatments, all completed services are non-refundable. 10. Right to Refuse Service Transform by Mon reserves the right to refuse or discontinue treatment.
